What we collect

- Name date of birth, GP details, medications and employment status
- contact information including email address telephone number
- demographic information such as address & postcode
- Basic information regarding your reasons for therapy at this time and any relevant background information that you feel will be useful in our working together.
What we do with the information we gather
We require this information to understand your needs and provide you with a better service, and in particular for the following reasons:
- Internal record keeping.
- We may use the information to improve our services.
- We may periodically send promotional email about workshops which we think you may find interesting using the email address which you have provided.
- From time to time, we may also use your information to contact you regarding workshops for psycho education and personal or professional development purposes. We may contact you by email phone or text.
Safeguarding
While the utmost serious consideration is given to protect the security and confidentiality of your personal information there are occasions where this confidentiality would be breeched for the following reasons. Risk of harm to self or others, where there are reasonable concerns that a child or young person maybe at risk from harm or neglect. These concerns, where possible would be discussed with you and with Julia’s supervisor, whilst maintaining your anonymity. Following this appropriate action would be taken.
Security
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect online and all paper records. Records are stored on either a password protected laptop or paper record which is stored in a locked filing cabinet.
Unless otherwise stated personal records are kept for 6 years and then confidentially destroyed.
